Private-practice requests also ask which offering interests you, your state and country, optional phone number, and permission to contact you about your inquiry and future availability. Newsletter signup asks for your email address. Please use your patient portal, rather than the general inquiry form, for medical records or sensitive health information.
The services involved
General contact messages are delivered through FormSubmit. Newsletter subscriptions are processed through Mailchimp. The homepage film is served directly by this website. Links to Fullscript, Charm, and the other websites in my work open those services, whose own privacy practices apply. Hosting providers also process the technical information needed to deliver and protect this website.
How information is used
Inquiry details are used to respond to your message. Newsletter information is used to send the communications described when you subscribe; you can unsubscribe using the link in those emails.
